The M Health Fairview Center for Community Health Equity runs this 12-month leadership fellowship for community leaders with an idea for a project that advances health equity and tackles the social determinants of health. Eight fellows are selected for the 2027 cohort, and each gets up to $10,000 to build, test and launch their project with coaching and peer support. You do not need a fully developed project to apply, and projects must be carried out in Fairview's service area in Minnesota and western Wisconsin. **What you get:** - Up to $10,000 in project funding to implement your health equity project - A $3,000 stipend to support your participation, for example compensation for time, transportation or childcare - Hotel accommodation and mileage covered if you travel more than 65 miles to in-person sessions - Coaching and technical assistance from field experts - A curriculum on health equity, program development and implementation - A cohort of eight peers and networking with professionals aligned with your interests **What you'll do:** - Attend about 105 hours of sessions from January to December 2027, usually two a month, mixing full-day in-person sessions across the 7-county Twin Cities metro area with 2-hour virtual sessions - Build, test and refine your health equity project over the year, then implement it with your project funds - Work through themes including funding and evaluation, project management, strategic communications, community engagement and policy, systems and environmental change - Present your work at a celebration and showcase in December 2027 **Key dates:** - Applications close Thursday, October 1, 2026, at 11:59 p.m. Central time - Semi-finalists notified late October 2026 and interviewed in early November (in person, virtual option available) - Final cohort announced mid-November 2026 - Fellowship runs January to December 2027, starting with orientation on January 28 and 29, 2027 **More information:** - The fellowship is for individuals, not organizations, and is not meant to replace an organizational grant or fund ongoing organizational work - Project funds can go to a fiscal agent of your choice or be held by a Fairview partner organization; the $3,000 stipend is reported on a 1099 - The application needs two one-page letters of recommendation and a draft budget on Fairview's template; if your project is affiliated with an organization, one letter must come from it - Questions go to CCHE@fairview.org
